Personal Care Consultants Ltd provides social work reports and occupational therapy reports for use by the courts. Our expert witness and consultancy services are used in clinical negligence, personal injury litigation and community care fields.

Our comprehensive social work reports are commissioned for the High Court by solicitors in relation to judicial review proceedings as well as Declaratory Relief proceedings in the High Court/Court of Protection instructed through the Official Solicitor.

We also undertake independent community care assessments and produce independent social work reports and investigations on behalf of the statutory services following complaints.

In terms of clinical negligence and personal injury litigation our expert witness consultants specialise in providing occupational therapy reports for individuals who have complex needs such as acquired brain injury, spinal cord injury, sensory impairment and behavioural disturbance.

Working in the domains of manual handling and physical intervention our experts are best placed to underpin their generic professional assessments by specialist manual handling and physical intervention risk assessments which predicates the assessment of care and support and enables assessors to identify measures to manage risk. Experience of current social and health care practice informs their assessment process.

Personal Care Consultants engages highly experienced professionals from social work, occupational therapy and nursing backgrounds. This allows us to adopt a multi-disciplinary approach to assessment and report writing where required.

Care Needs Assessment Reports social work reports identifying and costing care and support needs form birth to end of life. Best Interests Reports assessments in relation to the Mental Capacity Act and Deprivation of Liberty Safeguards to produce advice and recommendations for the Court of Protection and/or Supervisory Bodies in relation to future residence, care and support, contact issues and, where necessary, restrictive practices used to provide care and support

Our most commonly commissioned personal injury and clinical negligence reports are:

Occupational Therapy Reports – identifying measures in terms of specialist accommodation, equipment, adaptations, transport and service provision

Nursing Care Needs Reports – where there are specialist nursing care or medical needs

Specialist manual handling and physical intervention risk assessment reports often used by other expert witnesses to underpin their assessment of care needs and identify measures to reduce risk in terms of client / support ratios when there are manual handling issues of oppositional and complex behaviour

Initial Needs Assessment Reports – occupational therapy reports identifying immediate needs to progress the individual’s function, mobility, rehabilitation, safety and independence
